If you are no longer able to care for your dog and need help with finding him or her another home, please contact us for help. We will do our best to assist you. We will ask you to sign a surrender document, which gives us permission to re-home the dog on your behalf. We will also require a pet assessment to be completed so that we have all of the information we need to find your dog a good home. In addition to this basic information, we ask that you provide us with any medical records as well as photographs for your dog.
In order to ensure that your dog goes to a loving and suitable home, we complete home and reference checks for all of our applicants.
Please contact firstname.lastname@example.org for more information on surrendering your dog.
We may not be able to help in every case. We do not accept animals with aggression issues towards humans or animals. Before agreeing to taking in him or her, we will have a trainer visit you for an evaluation of the animal.
To get ready:
Gather all medical records.
If your dog is not up to date on vaccinations, please get that taken care of first.
Take some good pictures of your dog in clear daylight. Take some of the face and some of the body.
Create a short summary of your dog's behavior. Include everything - the good, the bad and the ugly.
Complete the two forms below
